Lifestyle United Weekend - Kids Program Registration

Welcome! We are excited to welcome your children to the Lifestyle United Weekend at Lifestyle Church.

We have created a safe, fun, and age-appropriate environment across the weekend with Glow Party Childminding, Movie Night Childminding, and our Sunday Kidstyle Program.

To help us prepare well and ensure every child is cared for safely, please complete one form per child and register for each session your child will attend.

Please note:

  • Check-in and check-out is required for every session on arrival.
  • No devices are permitted during children’s sessions
  • All children must be signed in by a parent or guardian
  • Child Minding & Sunday Kidstyle is available for children aged 18 months to Grade 6
  • Individual forms to be filled out for each child in family.

Friday Night – Glow Party Childminding

Check-in open: 6:45pm
Bring: Water bottle + jumper
No devices please
A fun glow-in-the-dark night with supervised play and activities.

Saturday Night – Movie Night Childminding

Check-in open: 6:15pm
Bring: Water bottle, pillow and jumper
No devices please
Food provided: popcorn and assorted chips
A relaxed movie night experience for kids.

Sunday Morning – Kidstyle Program

Check-in open: 9:15am
Includes: face painting + Kidstyle program
A vibrant, faith-filled kids experience for:

  • 18 months – Kindergarten
  • Prep – Grade 6

Permissions / Consent

Tip: When my child needs to go to the toilet, I give permission for a Kidstyle leader or team member, to escort my child to the toilet if required.

Tip: I being the parent/guardian of the said child hereby give my consent for my son/daughter to be captured in both photographs and video at Kidstyle/Lifestyle Church upon signing this form. Lifestyle Church reserves the right to use this material for promotional purposes. Lifestyle Church / Kidstyle may be filmed and/or recorded by or on behalf of Lifestyle Church for production and/or resale. It is a condition of entry that each audience waives any claim he or she might have in relation to inclusion of their likeness in such films or recordings.
